A minute (माइन्युट) is the written record of a meeting: who was present, what was discussed and what was decided. Every committee, cooperative, club, school management committee and company board in Nepal keeps one, and a bank or a government office will ask for a copy of the decision — निर्णयको प्रतिलिपि — before it opens an account, changes signatories or accepts an application from an organisation.

Opening line. The date, time and place, the kind of meeting, and who chaired it.
उपस्थिति (attendance). The name and position of everyone present, each with a space to sign. Invitees and observers are listed separately.
प्रस्ताव (agenda). The items discussed, numbered.
निर्णय (decisions). One decision for each agenda item, with the same number.
Closing. A line closing the meeting, then the signatures of the person who wrote the minute and the chair, and the organisation's stamp.
Writing a decision an office will accept
Say exactly what was decided, who will do it and by when. “Decided to open a savings account at Nepal Bank Ltd., Tokha branch, to be operated jointly by the Chairperson and the Treasurer” can be acted on; “discussed opening an account” cannot.
For a bank, name the bank and branch, the type of account and the signatories by name and position.
Keep the original in the minute book (निर्णय पुस्तिका). What you hand over is a copy, certified with the organisation's stamp and the signature of the chair or secretary.
Where a minute is asked for
Opening or changing a bank account, registering or renewing an organisation, applying to the ward office in the organisation's name, taking a loan, and appointing an auditor. When the minute goes with an application to a ward office, the निवेदन tool writes the covering application.

What is the format of a minute in Nepali?

Who signs the minute of a meeting?
Everyone present signs beside their name in the attendance list. The person who wrote the minute and the person who chaired the meeting sign at the end.
What is the difference between agenda and decision in a minute?
The agenda (प्रस्ताव) is the list of matters put to the meeting. The decisions (निर्णय) record what the meeting resolved on each of them, numbered to match.
