- Name, registration and contact — NMC registration number under your name, then phone, email and city.
- Profile — Qualification, years since graduation, the settings you have worked in, and your plans — for example, MD entrance.
- Clinical experience — Each hospital with dates, department, and what you managed: OPD numbers, emergency, inpatient, on-call.
- Internship and rotations — Hospital, departments rotated through and months in each.
- Education — MBBS with college, university and year, and any postgraduate degree.
- Trainings, research and presentations — BLS, ACLS, PALS, and any papers, case reports or presentations.
- Skills and references — Procedures you do independently, languages, and two senior doctors who can vouch for you.
