Asked for constantly, and almost never to hand when it is: a passport application where one parent cannot attend, a school trip abroad, a child flying with a grandparent or an aunt. Immigration officers, airlines and embassies all want the same few facts, set out clearly and signed.

What the letter has to say

Who the child is. Full name, date of birth, and the passport or birth certificate number.

Who is consenting, and on what basis. Parent or legal guardian, with citizenship or passport number and a phone number that will actually be answered while the child is travelling.

Who the child is travelling with. Name, relationship, and passport number — the escort will be asked to produce it.

Where, when, and why. Destination, purpose, departure and return dates, and the address for the stay.

Authority for medical treatment. The clause people forget and the one that matters most: it lets the accompanying adult consent to emergency treatment. It is in the template by default.

Before you rely on it

Many embassies and airlines want the signed letter notarised. That cannot be done here — print it, sign it in front of a notary, and carry the notarised original.

Some authorities want both parents to sign even when only one is travelling. The second guardian section is there for that; fill it in and both signature blocks appear.

Carry photocopies of both parents' citizenship or passports, and the child's birth certificate, with the letter.

Check the specific requirement of the embassy, airline or school first. They differ, and the letter is easier to get right the first time than to redo abroad.

Common questions

Does the consent letter need to be notarised?

Often, yes. Requirements vary by embassy and airline, and notarisation is what most of them mean by a valid consent letter. Print, sign before a notary, and travel with the original.

Do both parents have to sign?

Many authorities expect it, particularly for a passport or a visa. If only one parent is available, some require evidence of sole custody or of the other parent's absence — ask the specific authority.

Is this needed for travel within Nepal?

It is generally asked for at international borders and by airlines. Some schools ask for their own consent form for domestic trips, which this will not replace.
